It is usually the result of a contact of a body with a source
of energy (i.e. Kinetic, Electrical, Chemical, Thermal, etc.) above
the threshold limit of a body or structure.

It is an undesired event, which, under slight
different circumstances, could have resulted in harm
to people, damage to property or loss to process.

                                                    Near-Miss

An event that may or may not result into a loss.
Incidents with no visible injury and/or property damage.

               Preventable Accident

Preventable Accident is one which the worker failed to do everything he reasonably could have
               done to prevent it.

               Non-Preventable Accident

Non-Preventable Accident is one which occurred in spite of the fact that the worker did everything
               reasonable to prevent it.

               Accident Prevention Formula

                   1. Identify the Hazard
                   2. Understand the Defense
                   3. Act-in-Time
