               Case Study #1
                       Two men were working near the end of a scaffold about 15 feet off the ground. John told Henry to
               move out of the way so that he could more easily saw a board.  Henry back up without looking fell off the

               end of the scaffold, and broke his left ankle when he hit the ground.

               Case Study #2
                       A production operator sustained a fracture on two fingers of her right hand when she activated
               the 2-hand control push buttons of a hydraulic press with her elbow while placing a work piece into the
               press.


               Case Study #3
                       An electrician was disconnecting an electro-magnet power cable joint in order to remove the
               magnet from the crane hook.  However, when the cable joint was disconnected, an electrical flash-over

               occurred and the electrician sustained burns on both his hands.

               Recommendation for Corrective Action
                       Recommendation must state what specific action will be taken, by whom and when; and how the
               supervisor will assure that it is carried out correctly.
